Housing Is Too Big
Many keystone wallplates pack the openings together in a fairly high-density pattern. Most keystone inserts are only as wide as the opening they fit into, allowing them to be packed together in that dense space. Not so with this one. The back housing overlaps adjacent "cells", so that you can't place two side-by-side in a standard decora housing.
The part I don't get is *why* it has to be so big. It's just three wires connected to a 3.5mm tube. What's taking up all the space?
Also in the awkward design department, the wiring terminals require the wires to enter from the side of the housing, rather than the back. Again, this makes it difficult to pack multiple units side-by-side.
Back to the drawing board, guys.

on the 37036 .5 mm jack what wall plate works ? I have several different plates and surface mounts but none will allow a good fit? thanks

These terminals will accept up to 22 AWG.
